Labels do not forgive manual gaps. An industry analysis of 2024 FDA recall data found that labeling errors accounted for roughly 45% of US recalls, with an estimated $1.92 billion in direct losses. A label gets designed and approved before a run ever starts, which makes that number harder to accept, and harder to explain to leadership after the fact.
You already know the steps a label has to clear. The failures rarely happen because someone skipped a step. They happen in the handoffs between steps, where ownership gets fuzzy and everyone assumes someone else looked. Here is where those breakdowns show up, and how to close each one.
The five places a label error slips through

1. Label design and artwork changes
Most errors start here, at the design stage or when someone makes a change. A formula change means you re-verify the ingredient statement and allergen declaration against the new recipe. A marketing refresh that does not touch the formula still needs a check, because a redesign can move, shrink, or drop mandatory information without anyone noticing.
Build the control into design itself. Use a checklist or a digital tool that confirms every piece of mandatory information is present, in the right location, at the right size. Then confirm the optional claims. You cannot carry a USDA organic seal or a gluten-free claim you cannot document, so the person creating the label should hold the proof before it goes anywhere.
2. Ordering and proofing with the printer
When you send artwork to a printer, two things have to match: the version number you approved and the version the printer loads into their portal. Review the proof in that portal if you can. Printer formatting can regroup or hide information, and mandatory elements have to stay separated and fully legible. An allergen statement that gets visually merged with the ingredient list is a failure, even when every word is technically present.
3. Verifying label rolls when they arrive
When rolls arrive, check them against your digital master before they touch inventory. Confirm the version number and every piece of mandatory information, allergens first. Log the roll and its version into your system, and require a confirmatory signature at receiving. This is the cheapest place to catch a printer error, because the labels have not gone near a product yet.
4. Loading the wrong version on the line
This is the one that bites hardest. When Tony polled attendees on where errors are most likely to slip through, 54% said the wrong version getting loaded on the line. It reads more as a communication problem than a labeling problem; operations does not always know a label changed for the next run.
Give operators a single source of truth at the equipment: a placard or an on-screen production schedule that shows the exact formula and label version running. If you run more than one version of the same formula, mark a clean lot change and update the placard at the same moment. When a run uses one version, move every other version of that product into a locked or quarantined cabinet so nobody grabs it by mistake.

5. Final verification before release
Before finished product ships, your label team verifies the label on the product against the batch record. You are likely already pulling retains for finished-product testing, so fold a label check into that step: mandatory information, optional information, and the version against what the scheduled run called for. Keep a record of every label used, including notes on what changed.
Link every label version to a formula version
The control that holds the other five together is version linkage, and it is where most teams fall short. When Tony asked whether label versions are formally linked to a formula or spec version, 52% said they track versions but not the linkage.
Here is the fix: every label version should connect to a formula version in your records, whether that lives on paper, in your ERP, or in your production documentation. One formula version might have three approved label versions that differ only for marketing reasons. If all three trace back to formula version six, running any of them is not automatically a recall event when the formula itself is correct. That linkage doubles as a built-in check: any time a formula or a label changes, you are forced to look at the other one and confirm the connection still holds.
When you switch a label version, document what changed. A revision number tells you which one you are holding. A change note tells you why, so you are not guessing months later when someone asks.
Treat label verification as change management
If a director could make one change on Monday, Tony's answer was direct: build a cross-functional sign-off system and run label verification as change management.
Label verification reaches across the plant. It touches R&D, who own the formula and should sign that it is correct. It touches operations and scheduling, whose batch records and production paperwork have to name the right revision. It touches marketing and sales, who need to know what is running and when. Bring the label team in any time something changes, and add the relevant departments to a formal change-management process for label changes.
There is a practical trick for keeping that sign-off from becoming a rubber stamp. Assign each department a specific part of the label to review. One person always checks allergens. Another always checks net weight. Nobody signs off on the whole label at a glance, which is how diffusion of responsibility lets a run of small misses stack into a recall.
